POSITION SUMMARY: The Brokenhead Ojibway Nation Health Center is seeking a highly motivated individual to fill the full-time term position of Home & Community Care Nurse Coordinator.  Under the general direction of the Health Director, will provide nursing services, within defined scope of practice, to clients as well as providing the day-to-day operations of the Home and Community Care (H&CC) Program, including personnel and program management.

QUALIFICATIONS AND SKILLS REQUIRED:

  • Diploma – Registered Nurse, Licensed Practical Nurse registered to practice in Manitoba
  • Previous supervisory experience in health care
  • Previous nursing experience in a First Nations setting
  • Excellent training and motivational skills
  • Financial management experience an asset
  • Self-directed self-starter
  • Demonstrated ability in public relations and interpersonal skills
  • Good oral and written skills
  • Ability to speak Ojibway is an asset

NURSING DUTIES:

  • Assist individuals living with chronic and acute illness to maintain optimum health and independence in their home and community
  • Ensure all clients who require home care services have access to services in the community, where possible
  • Involve the client, family, other caregivers and/or service providers in client assessments and reassessments
  • Develop, implement and evaluate care plans
  • Case manage and refer to existing services
  • Conduct home visits to provide nursing services, within the defined scope of a Licensed Practical Nurse to clients, based on the client assessment and care planning procedures
  • Observe and document therapeutic effects of medications
  • Provide pre-operatives and post-operative personal and comfort care
  • Monitor and report client progress, evaluate effectiveness of nursing interventions and consult with appropriate members of the healthcare team
  • Manage medical supplies and equipment
  • Explore and submit the documentation to the Long-Term Care (LTC) Access Centre Panel Review Board for other options for care such as assisted living, supportive housing and companion care for clients whose needs can no longer be met by the program
  • Provide changes of client care in writing to the client, family and support staff
  • Provide support, safety and health education to clients and their families
  • Maintain information and data collection for monitoring, planning, reporting and evaluation of activities
  • Accept Clinical Supervision to reflect on and review program practices, discuss individual cases in depth, change or modify program practice and identify training for continuing developmental needs
  • Promote, and assist with, community recreational and cultural activities that foster a healthy community
  • Submit written monthly reports to the Health Director, as well as regularly scheduled financial and narrative reports to funding agencies
  • Participate in regularly scheduled meetings with the Community Health, Resource and Accreditation Meetings
  • Utilize Electronic Medical Record to document client files

COORDINATING AND SUPERVISORY DUTIES:

  • Develop and implement an annual workplan and budget that relates to the Health and Wellness Strategic Plan
  • Build capacity with Health Care Aides and Home Support Workers to deliver comprehensive, culturally sensitive, and effective home care services
  • Provide Clinical/Reflective and Administrative supervision with program staff
  • Manage the delivery of a quality services in a safe and effective manner
  • Direct all day-to-day operations for the program, including personnel, program, and financial management
  • Assist in the development of policy, inter-agency liaison, office organization and personnel selection
  • Consult with Community and Leadership upon making changes to the Home Support Program
  • Organize staff training, workshops and capacity building and attend training sessions as needed
  • Complete scheduled payroll requirements and personnel requirements
  • Ensure that the H&CC Program meets all professional, legislated and/or regulatory requirements
  • Prepare and present reports to the Health Director, Chief and Council and the community
  • Ensure that all client confidentiality and proper consent is maintained
